Wright State Physicians Department of Geriatrics is now offering a Senior Assessment Geriatric Evaluation (SAGE) Clinic. The SAGE Clinic offers an assessment program for those 65 years of age and older, addressing concerns of older adult-related conditions.
“As the only practice in the Dayton area solely devoted to the care and welfare of older adults across the spectrum of care, Wright State Physicians geriatrics has developed the SAGE Clinic to provide a comprehensive geriatric assessment for those who have concerns about function, quality of life, and conditions such as progressive memory loss, new onset confusion, medication overuse, and falls, as well as any other significant concern,” Dan Swagerty, M.D., M.P.H., professor and chair of geriatrics with Wright State University Boonshoft School of Medicine, and physician in the Department of Geriatrics at Wright State Physicians.
The SAGE Clinic’s procedure involves a two-step process in the clinic with the Wright State Physicians geriatric interprofessional team.
The first step in the process is to assess the older adult and their support person or caregiver. The older adult’s function, quality of life, medications, medical and other older adult-related conditions, including evaluating the need for community resources.
The second step involves the older adult and their support person or caregiver receiving a comprehensive set of diagnoses and recommendations about treatments, along with information about community resources.
A full report of the SAGE Clinic assessment is provided to the older adult’s primary care clinician.
The SAGE Clinic is available by referral from medical providers, community agencies, families, or self-referral.
If the older adult desires a comprehensive, but not team-based approach, Wright State Physicians Department of Geriatrics can provide a one or two visit consultation in the clinic. These providers are accepting new patients for their primary care geriatrics clinics.
All home and clinic visits are conducted under the strictest COVID-19 precautions, as recommended by the State of Ohio and the Centers for Disease Control and Prevention.
