Your Office Visit

Office visits are scheduled between the hours of 8 a.m. and 4:30 p.m. When calling, please indicate the reason for your visit so we can make your appointment with the appropriate specialist. If you cannot keep your appointment, please let us know 24 hours in advance. After hour physician on call coverage is provided.

What to Wear

If you are visiting due to an upper extremity (shoulder, arm, wrist) injury, please wear or bring a tank top or sleeveless blouse. If you are coming to our office due to a lower extremity (thigh, knee, calf, ankle) injury, please wear or bring shorts.

What to Bring

If you are mobility impaired, be sure to have someone accompany you. Bring the names of previous procedures (if any) and a list of current medications.

Patient Discount at the Dayton Marriott Hotel

For your convience, the Dayton Marriott — partnering with Miami Valley Hospital — provides a discount to any patient having surgery or visiting the hospital for a patient appointment. Located one block from the hospital, the Dayton Marriott is a practical option if distance or weather is a concern for you or any of your family members. Please contact the Dayton Marriott at (937) 223-1000 to reserve a discounted room during your visit. Please present this flyer at the Marriott front desk when you arrive.

Confidentiality and Your Records

Both your medical care and records are treated in a highly confidential manner. No information will be released to an insurance company and/or third party without a written authorization. If you would like a copy of your records, a 48-hour notice will be required. There may be a slight charge.

Billing Information

In order for us to file your insurance claim correctly, it will be necessary to make a copy of your insurance card when you arrive for your appointment. Copayments and deductibles are due at the time of service. For your convenience, we accept VISA, Mastercard, American Express and Discover.

If your insurance coverage requires you to have a written referral/precertification from your primary physician before seeing one of our specialists, you must present the document upon arrival for your appointment. Failure to do so will automatically make you responsible for charges.

Disability and Workers' Compensation

Workers' Compensation cases must include a claim number and verification of any work-related injury or condition. It is always the patient's responsibility to provide the physician with the necessary forms to complete claims. Workers' Compensation and Disability Forms require additional time and personnel to complete. Please allow seven to ten working days for completion of this paperwork, or you may ask the office to contact you by phone when the forms are ready for pick-up.

Failure to provide accurate information may result in your liability for the entire physician's bill. You are always responsible for the balance of your bill not covered by insurance. There is a $10 completion charge, payable in advance, for all disability forms except Workers' Compensation.

Prescription Refills

Please call (937) 208-2016 for prescription refills. Prescription refill requests received prior to 3:00 p.m. will be phoned in to the patient's pharmacy by 5:00 p.m. the same day (with physician's approval). Requests received after 3:00 p.m. will be handled on the next business day (with physician's approval). Any messages left on weekends or holidays will be handled on the next business day.
